**NEW** You will be required to correctly answer a skill-testing question in order to claim your prize.

You must claim your prize in person and must provide photo ID before prizes may be released.  If you won through the web, you must also bring a copy of the email you have received from JACK FM Promotions.  If you want someone else to pick up your prize, they must come with a letter from you and a photocopy of your official ID.  The letter must name who will be picking up your prize and include their home telephone number.

Please note the prize you have won and the date of winning.  You have 30 days from the day you won on-air or the day the winning email was sent to you, to pick up your prize.  Please be advised that some prizes have time restrictions(eg. concert tickets or movie passes)and it is the responsibility of the listener to pick up their prize before the expiry date.  If you pick up a prize after it has expired, JACK FM is not responsible for replacing it.

Some prizes will not be awarded to winners who are under 18 years of age.  In some specific cases (eg. trip winners), parents or legal guardians will be required in-person to sign a waiver form on the winner’s behalf.  Please be aware our station policy is that listeners are not permitted to win more than once in a 30 day period.

All prizes are non-transferable, non-refundable, and have no cash value.  There will be no substitutions
